I. Introduction
You are reviewing and acknowledging this Telehealth Informed Consent because you are seeking healthcare services through telehealth technologies provided by licensed healthcare professionals via the Fit and Glam platform. This includes our website, mobile interfaces, or any digital tools collectively referred to as the “Fit and Glam Platform.” This Telehealth Informed Consent supplements, and does not override, the Terms and Conditions, Privacy Policy, or other agreements between you and Fit and Glam or your Provider.
By booking a consultation, clicking “I consent to telehealth,” checking a related acceptance box, or using any other method of acceptance, you confirm that you have reviewed the risks and limitations described herein and consent to receive services via telehealth technologies. A copy of this consent will be maintained in your medical record, and your continued use of Fit and Glam’s telehealth services represents an ongoing acknowledgment of your consent.
II. What is Telehealth?
Telehealth involves the use of digital information and communication technologies (e.g., computers, smartphones, video conferencing, secure messaging) to provide clinical healthcare services remotely. It may include real-time video/audio consultations, asynchronous (store-and-forward) messaging, or interactive tools. These services may include diagnosis, treatment plans, prescription issuance, follow-up care, and patient education.
III. Benefits of Telehealth
- Convenient and timely access to medical care without the need to visit a clinic.
- Accessibility to licensed medical professionals regardless of geographical location.
- Efficient follow-up care related to treatment plans.
IV. Risks and Limitations of Telehealth
- The information you provide and that is transmitted may not be sufficient for accurate diagnosis or treatment.
- Technology issues (e.g., poor internet connection, system errors) may interrupt or delay consultations.
- Not all conditions or symptoms are appropriate for telehealth evaluation. Providers may determine whether in-person evaluation is needed.
- Data security measures are in place, but there remains a small risk of unauthorized access to your personal health information.
- Misdiagnosis or treatment errors may occur due to incomplete data or technical limitations.
V. Diagnostic Testing and Third-Party Services
Certain diagnostic or laboratory tests may be required by your healthcare provider. These services may be provided by third-party laboratories, and Fit and Glam and your Provider cannot guarantee the accuracy of these test results. These tests may produce false positives, false negatives, or inconclusive results, potentially affecting your care.
VI. Patient Acknowledgments
By accepting this consent, you acknowledge and agree that:
- You understand the risks, limitations, and benefits of receiving healthcare via telehealth.
- You’re aware that Telehealth increases the risk to the privacy of your health information.
- Your Provider is a Filipino licensed physician working in collaboration with a Nutrition and Dietitian and Physical Fitness Coach for a whole-person care approach.
- Non-provider staff or translators may be present during your consultation to assist with technology or language, and you will be notified of their presence.
- You certify that all information you provide is accurate and complete.
- Your Provider may determine it is appropriate to use asynchronous (store-and-forward) methods of care during consultations and follow-up consults.
- There is no guarantee that a prescription will be issued. This decision rests solely with your Provider’s professional judgment. You also understand that no such benefits or specific results can be guaranteed.
- You hold Fit and Glam and its associated healthcare providers harmless for delays or failures in service due to technical issues.
- Diagnostic or laboratory services provided by third parties may have limitations and Fit and Glam is not responsible for the accuracy of these results.
- Fit and Glam offers specific services only. Other medical needs may require additional providers or services. There is no guarantee that you will be approved for treatment by a Provider. Fit and Glam’s Healthcare Providers reserves the right to deny care for any reason if, in their professional judgment, the provision of the services, including when provided via telehealth is not medically or ethically appropriate.
- Your consultations may not be in real-time; response times may vary, and delayed communication through the platform is expected.
- You may withdraw consent for telehealth services at any time; however, completed consultations are still billable.
- You may access your medical records in accordance with applicable data privacy laws.
- You may choose any pharmacy or lab for your prescriptions or diagnostic tests.
- Fit and Glam is a third-party beneficiary of this Telehealth Informed Consent and reserves the right to enforce its provisions.
By continuing to use the Fit and Glam Platform or initiating a consultation, you affirm your understanding and acceptance of this Telehealth Informed Consent.
